Patient Assistance Program Review
As part of our mission to the community we live in, Live Oak Pharmacy is committed to help qualifying patients without prescription drug coverage get the medicines they need for free or nearly free. This is a 20-minute consultation.
How Do I Start?
- Set up your consultation with one of our specialized staff members. Before your consultation you'll spend some time filling out your Medication Record in the convenience of your own home.
- During your 20-minute consultation, we will help you determine whether you may qualify for a prescription assistance program by searching for your medications and answering some short questions.
- If you are eligible for any programs, our specialized staff member will help you fill out the necessary paperwork. Some program applications require that you contact the pharmaceutical company; others may need to be filled out and signed by your doctor.
Consultations are offered on Tuesday and Wednesdays between 10:00 a.m. and 6:00 p.m. or by appointment on Monday, Thursday, Friday or Saturdays.
